Illinois 5Essentials Survey 2019-2020
Starting on Monday, October 28, 2019 teachers, parents, and 4th – 8th grade students began filling out the 5Essentials Survey. This survey is a comprehensive, evidence-based tool designed to drive improvement in schools. The survey will generate a detailed picture of the inner workings of our school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school. The survey takes about 10 minutes to complete.
The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. The five indicators that affect and predict school success are:
We need at least 20% of Evergreen parents and 20% of Benjamin parents to complete the parent survey to generate a parent report for each school. If you have students at both schools, please complete a survey for Benjamin and Evergreen. If you have more than one child and they attend the same school you may participate multiple times. Only one parent per household can participate.

Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
Note: Parents have the option to leave their email address so that we can send them confirmation of survey completion upon submission of the parent survey supplement. That text box can be left blank if the parent doesn’t want to provide or have an email address.
The parent survey will take about 10 minutes to complete. To help us reach our 20% goal of parents taking the survey, we thought it might be helpful if you arrived 10 minutes early when volunteering so you can complete the survey.
